Needing 290 for a 2-0 series lead, England lost their last six wickets for just 36 runs, subsiding to 263 all out.
Jos Buttler and Tom Curran were dismissed in consecutive balls by Holder, while Sheldon Cottrell took 5-46, a career-best.
Earlier, Shimron Hetmyer was 104 not out, contributing to West Indies' 289-6.
Eoin Morgan and Ben Stokes left England in a comfortable position after a 99-run stand for the fourth wicket, needing just over a run a ball.
Captain Holder brought himself back into the attack in the 35th over with England 100 away from victory, this decision changed the game.
Stokes fell first, edging a full Holder delivery to wicketkeeper Hope for 79.
In the next Holder over, Buttler skied an off-cutter to Hetmyer, before a Curran lbw which, on replay, was comfortably wide of leg stump.
Stokes had used England's only review on his dismissal and, with the run-rate climbing, the tourists never recovered.
Cottrell had Adil Rashid caught behind and two balls later bowled Moeen Ali, before Liam Plunkett drove Carlos Brathwaite to long-off.
Holder took the catch to seal his team's first one-day win over England in five years.

Shimron Hetmyer's 83-ball century was his fourth in ODIs in the past year, it helped West Indies recover from a quiet middle-over period, striking seven fours and four sixes.
Nurse and Hetmyer shared an unbroken 52-run stand in 5.3 overs, with the latter drilling down the ground to reach three figures off the penultimate delivery of the innings.
Opener Chris Gayle made a 50 which included four sixes before he was bowled aiming to hit leg-spinner Rashid across the line.
Rashid ran out Darren Bravo for 25 with a direct hit from short third man.
Jason Roy also produced a brilliant throw, running to his right from backward point and aiming at one stump, to dismiss Holder.
